Fees, Insurance, and Finances ...

Patients are responsible for payment of services rendered including co-payments, deductibles, and all other fees, at the time of service. We accept cash, check, money order, Visa and MasterCard. There is a $10.00 fee for co-pays that are not paid at the time of service. There is a $25.00 fee for returned checks. Please remember to bring your insurance cards with you for all visits Please inform our staff of any changes in insurance, home address, phone numbers, and employment. While your insurance coverage is a contract between you and your insurance company, payment for services is your responsibility. Questions regarding fees should be directed to our bookkeepers.  Our insurance coordinators are specially trained to process and file insurance. They will be happy to assist in any way they can.
